    button at the bottom of the email communication. It may take up to ten (10) days for this to take effect.

Security. Proactive Health Solutions, LLC (PHS) is not responsible for the security or HIPAA compliance of the apps, links, or connection of third-party services found on PHS owned iPads, websites, newsletters, etc. Please contact the appropriate app or website developer regarding any security or HIPAA questions. If you are using an iPad for healthcare services, you may want to ask the provider about security and HIPAA compliance. Users are required to remove personal data from PHS iPads by resetting the device when finished. Proactive Health Solutions, LLC, is not responsible for personal information left on the device. If you have questions on how to reset the device, then please email health@phs-ohio.comWe take the security of Personal Information seriously and use reasonable electronic, personnel and physical measures to store and protect it from loss, theft, alteration, or misuse. However, even the best security measures cannot fully eliminate all risks. We cannot guarantee only authorized persons will view Personal Information. We are not responsible for third-party circumvention of any privacy settings or security measures and We cannot ensure or warrant the security of any information You transmit to Us on Our website. Therefore, We are not responsible for any actual or consequential damages due to a lapse in compliance with this Policy because of a security breach or technical malfunction.

How to Complain. We hope to resolve any query or concern You raise about Our use of Your information. Complaints may be lodged with a supervisory authority in Your resident state or country or where any alleged infringement of data protection laws occurred. The supervisory authority in the country is the entity who may be contacted.

Changes to this Policy. This Policy updates the old policy and was last published on 4.12.24.

We reserve the right to modify this Policy at any time. When We do, We will inform You by posting a “last updated” notice to the Privacy Policy section on Our website. Changes to this Policy will take effect immediately upon posting.

We treat the personal information of our customers in a confidential manner. We do not provide any information to unrelated companies for the purpose of marketing their products or services to our customers.

Passwords:

To use certain features on www.phs-ohio.com, you may need a username and password. The responsibility for maintaining the confidentiality of your login information and all activities that occur while your account is logged on. Also, it is your duty to notify us if your login information was or could be jeopardized and to exit from your account when finished to prevent the next user from gaining access. We are not responsible for any losses or damages that arise from you not protecting your login information. 

 

Unsubscribe:

You may unsubscribe to our emails and newsletters by unsubscribing using the link at the footer of all newsletter communications, or by sending us an email. We manage our newsletter lists via a list management system. You may unsubscribe from one list managed, but doing so may not remove you from all publication newsletter lists. Facebook: We post links to new stories and other alerts at the PHS Health Facebook page. If you own a Facebook account or 'Like' our Facebook pages, you can post comments or click on the 'like' option for your entries. If you comment or click on the 'like' button, personally identifying information will be visible to PHS staff and other Facebook site guests. The amount of viewable personal data will depend on your individual Facebook privacy settings. You can fully avoid displaying any personally identifiable information by not producing an account, not posting remarks, and not selecting the 'like' options on Facebook. PHS staff does not collect, use, or disclose any information about visitors who comment or 'like' the PHS Facebook sites. Facebook gathers and reports on non-personally identifiable information about activities on Facebook pages. This data is password protected and only available to PHS managers. Twitter: PHS uses Twitter to send short messages or 'Tweets' (up to 140 characters) to share information about wellness with you and respond to your comments and inquiries posted via Twitter to PHS. While you may read the PHS Twitter feeds without subscribing to them, if you want to subscribe to (or follow) PHS Twitter feeds, you must produce a Twitter account at www.twitter.com. To create an account, you need to provide personal data, such as your name, username, email address, and password. You have the option to provide extra personal information, including a biography, picture, or location. Most information you provide for a Twitter account is available to the public, but remember you can modify how much of your data is visible by making changes to your privacy settings at the Twitter.com Web site. PHS staff members monitor the number of subscribers and reply to remarks and questions via Twitter, but PHS staff never take possession of the personal details belonging to you as a Twitter follower. However, as a practice, comment moderator policy demands the removal from the PHS Twitter pages all comments that contain spam or are inflammatory, inappropriate, or obscene.
