Proactive Health Solutions, LLC (PHS) is not responsible for the security or HIPAA compliance of the apps, links, or connection of third-party services found on PHS owned iPads, websites, newsletters, etc. Please contact the appropriate app or website developer regarding any security or HIPAA questions. If you are using an iPad for healthcare services, you may want to ask the provider about security and HIPAA compliance. Users are required to remove personal data from PHS iPads by resetting the device when finished. Proactive Health Solutions, LLC, is not responsible for personal information left on the device. If you have questions on how to reset the device, then please email health@phs-ohio.com. We take the security of Personal Information seriously and use reasonable electronic, personnel and physical measures to store and protect it from loss, theft, alteration, or misuse. However, even the best security measures cannot fully eliminate all risks. We cannot guarantee only authorized persons will view Personal Information. We are not responsible for third-party circumvention of any privacy settings or security measures and We cannot ensure or warrant the security of any information You transmit to Us on Our website. Therefore, We are not responsible for any actual or consequential damages due to a lapse in compliance with this Policy because of a security breach or technical malfunction.

To create an account, you need to provide personal data, such as your name, username, email address, and password. You have the option to provide extra personal information, including a biography, picture, or location.
